As a condition of employment or affiliation, all Memorial Hospital and Manor employees must be immunized against influenza during each defined flu season, or, if granted an exemption must comply with alternate preventative measures. New employees starting during the time that immunization is available but after November 1 must be immunized or granted an exemption before beginning to work.
Candidates or employees, who are immunized through services other than by Memorial Hospital and Manor, must provide written proof of immunization to the Infection Control Department. Proof of immunization may include a dated physician’s note, healthcare facility record, retail store record, public health clinic document or G.R.I.Ts verification of immunization.
