Registration

Memorial Hospital and Manor welcomes you as a patient at our hospital. We want your experience here to be as pleasant and comfortable as possible. Registration for all outpatient services is located next to the lobby area at the front entrance of the hospital.

Pre-registration

Some procedures may require appointments, so please contact the Registration Department at (229) 243-6299 to schedule any necessary appointments. Also, you or your physician’s office should contact our Registration Department at (229) 243-6270 to Pre-register for your visit. When you pre-register for your outpatient procedure, a registration clerk will prepare the necessary paperwork before you arrive, which will decrease the length of your wait.

Registration Procedures

ALL outpatient registration begins at the Front Lobby Registration desk.  Sometimes your physician may give you a “Centralized Registration Order Form” to bring to the hospital for laboratory testing, X-rays, or other diagnostic testing. Please read the special instructions regarding your exam which are printed on the back of the form.

What You Will Need To Bring For Your Outpatient Procedure:

  • Physician Orders
  • Insurance Cards
  • Authorization or Claim Forms
  • Social Security Number
  • Current Medication Information
  • Emergency Contact Information
  • A Photo ID

The following Outpatient Services require a physician order:

Laboratory

After registration forms are completed, you will be sent to the Laboratory where blood or urine samples will be collected.  Many patients who have routine blood work may find it more convenient to visit our satellite laboratory at Memorial Medical Center, 603 Wheat Avenue, between the hours of 8:00 a.m. – 12:00 p.m. and 1:00 p.m. to 4:30 p.m. on Monday through Thursday. No appointment is necessary during normal working hours.

Radiology (X-Ray)

Our Radiology Department offers advanced diagnostic testing including CT Scans, Magnetic Resonance Imaging (MRI), Mammography, Ultrasound, Fluoroscopy, Osteoporosis Screening and Nuclear Medicine. Your physician’s office will call to schedule an appointment time. If you have any questions regarding your tests, please call the Radiology Department at (229) 243-6180.

Cardiac Testing

Your physician may schedule diagnostic testing for you including echocardiography, stress testing, or Holter monitoring which are available in the Intensive Care Unit.

Rehabilitation Therapy

Memorial Hospital and Manor’s outpatient Rehabilitation Therapy Department is located at Memorial Medical Center, 603 Wheat Avenue. You or your physician may schedule an appointment for your initial assessment by calling (229) 243-8124.
