Patient Rights
As a patient at Memorial Hospital and Manor, you have rights and responsibilities to ensure you receive the best possible care. Those rights and responsibilities are outlined in a brochure that is provided on admission. Your
rights and responsibilities also apply to your guardian, next-of-kin,
or designated representative. For comments or concerns regarding your
patient rights, contact the Patient Representative at 229-243-6372 or SocSvc2@mh-m.org . You also have specific privacy rights that are outlined in our Notice of Privacy Practices.
If you have
concerns about the quality or care you receive or your safety while
here as a patient, please notifty the Patient Representative. You
may also ask to speak with the "House Supervisor," who is on duty 24
hours a day, 7 days a week. An Administrator is always on call to
assist in solving problems, and may be reached through the Hospital
Operator at (229) 246-3500. Concerns about patient care and
safety that are not resolved by the hospital administration may be
referred to the Joint Commission's Office of Quality Monitoring at
1-800-994-6610 or via email at complaint@jcaho.org. Concerns may
also be forwarded to the Office of Regulatory Services, Healthcare
Section, 2 Peachtree Street, NE, 33rd Floor, Atlanta, GA 30303,
or by phone at (404) 657-5726.
