Award Nominiation Forms: use this link to nominate an employee to receive an award during our annual employee picnic and awards program in May. Once you have printed and completed the forms, please send to the Human Resources Department by March 27, 2009.

Claim form for Colonial Insurance Company: use this link if you wish to access a claim form for Disability, Life, Accident, Sickness, and/or Cancer Insurance with Colonial Insurance Company. Instructions: Print blank claim form, complete the employee portion, have your physician complete the physician portion, and then submit the entire claim form to the Human Resources Department.
